Roper St. Francis HealthCare Diversity and Inclusion Health Equity Community Health Department was awarded the 2024 Outstanding CHW Program of the Year Award on Monday, October 28, 2024, at the CHW Impact Summit. The Roper St. Francis HealthCare Diversity and Inclusion Health Equity Community Health Department, CHWs serve the Tri-County's marginalized individuals and rural communities. This program has been impacting the community for 2 years now.

When asked what impact has this CHW Program made on the communities they serve?

The answer was:

The Community Health team encourages partnerships with key organizations that the social determinants of health (economic stability, neighborhood and physical environment and physical environment, education, food, community, and social context.

The Program spends countless hours in the community meeting individuals where they are. They are out on the mobile unit conducting health screenings, referring patients to supportive services, and scheduling patients with a primary provider.

This CHW Program is dedicated and committed to providing health education, addressing social needs, and providing services to three Tri-County Area counties.
