2024 Outstanding CHW of the Year

Marta Trantham of Tandem Health was awarded the 2024 Outstanding CHW of the Year Award on Monday, October 28, 2024, at the CHW Impact Summit. Marta has been with Tandem since March 2018 and is an amazing asset to the Tandem Health team as well as her Enabling Services departments. 

On the nomination form, it was said that Miss Marta is an open book of resources for our Tandem patients, especially for our Hispanic community, pediatrics, and OB/GYN patients. She's a Godsend to our patients who get a diagnosis of breast or cervical cancer, helping them with Medicaid apps. 

When asked: How has the CHW contributed significantly to improving health outcomes in their community?

The answer was:

She helps patients maintain their health needs whether it is for a child or one of our OB/GYN patients. For women needing female exams with no insurance, she assists with BCN apps making sure they receive their exam. As well as Welvista and Medicaid.

A further statement about Marta was: "Marta has a heart of gold, she is a team player, caring, an encourager, joyful, and a hard worker. She is always giving 100% of herself to the patients and this medical office. Every time you see her she is always smiling and ready to see what she can do to help. She loves giving hugs to her fellow coworkers when she sees you and if you are having a bad day she is that light that shines bright for Tandem to give you that push to keep going."
