Are you or your organization passionate about supporting individuals at risk of developing chronic diseases? The Centers for Disease Control and Prevention (CDC), in partnership with ICF, is calling for nominations to participate in an exciting new initiative aimed at enhancing the implementation of Lifestyle Change Interventions (LCIs).

About the Initiative
This project is led by the CDC’s Division of Diabetes Translation and seeks to evaluate innovative strategies to improve the enrollment, retention, adoption, and sustainability of LCIs. These interventions play a critical role in preventing chronic diseases by encouraging healthier lifestyles.

Up to 18 organizations will be selected to participate in this evaluation, and each selected site will receive a $5,000 token of appreciation.

Why This Matters
Community Health Workers (CHWs) and organizations like the South Carolina Community Health Worker Association play an essential role in addressing health disparities and improving health outcomes. This opportunity aligns with their mission to foster healthier communities.
